Dr. Tirrito, a cardiologist with Pima Heart, follows each of our cardiac patients multiple times each week. This is pivotal to the success of the program as it allows for much greater attention to clinical detail of the patient. Assessment are completed and problems addressed before symptoms become problems.
Pima Heart is leading the way in Cardiac Care in Southern Arizona. Our partnership with Dr. Tirrito and Pima Heart grants us access to a full array of resources necessary for any testing or procedures.
Our patient’s well-being is important to us even after discharge. One of our main goals is to prevent unnecessary readmissions to the hospital. In order to ensure a successful transition back to the community, the team meets weekly with a preferred home health agency to discuss our cardiac patient’s recovery. If a patient experiences a decline and would benefit from further skilled nursing and/or therapy services, we may be able to readmit that patient back to Sabino Canyon Rehabilitation within insurance and Medicare guidelines.
We have partnered with Pima Heart and other leading cardiac educators in the community to ensure our nursing staff is proficient on cardiac protocols and is able to detect early signs and symptoms before problems arise.
Dietary Staff offer heart-healthy diets and ensure each meal is in harmony with cardiac plan of care.
Physical therapy, which will include warm up, conditioning, and cool down exercise along with physiological monitoring. During therapy blood pressure, heart rate and oxygen levels will be monitored.
Occupational therapy, will help with menu planning and meal preparation techniques. Upper extremity exercises, energy conservation techniques, and education in the patient’s cardiac precautions will be reinforce during activities of daily living.
Speech therapy, will be available to assess for dysphagia and teach breathing exercises as well as assess cognitive levels to ensure that patients are properly educated.
